But, given the popularity of tooth whitening, it’s more important than ever to discuss it as a possibility. After all, sensitivity can’t be treated if it’s ignored.

However, before proceeding with any whitening or sensitivity treatment described below it is important to assess the cause and severity of your patients’ sensitivity. Be sure to conduct a thorough examination to rule out more serious causes such as decay or exposed dentin from bruxism. Such conditions will require treatment prior to whitening.

Reducing Tooth Sensitivity Before a Tooth Whitening Treatment

What They Didn’t Teach You About Selecting Loupes in School!

Selection Guide of Loupes

When we purchase loupes, we should consider several important factors such as loupe mounting styles, magnification power, field size, declination angle, frame design, nose pad options, and maintenance cost. These factors are evaluated based on working distance, working posture (sitting or standing), and facial features such as nose shape.

Selection of Loupe-Mounting Styles

There are two basic mounting styles for ergonomic loupes: customizable FLM (front-lens-mounted) style and custom TTL (through-the-lens) loupes (Figure 1).

1. If you do not know a desirable declination angle for your posture, you may choose the FLM style which allows you to try different declination angles to achieve your best neck posture.

2. If you know a desirable declination angle, you may choose the TTL style, but if your nose is flat, the FLM style may be a better choice to avoid fogging and touching your eyes.

3. If you are experiencing chronic neck pain, you may choose FLM loupes and adjust the declination angle gradually.

Selection of Magnification Power for Your 1st Pair of Loupes

The resolution power of eyes decreases as the working distance increases (Figure #2) and thus the required magnification power increases as the working distance increases. Desirable magnification power for the 1st pair of loupes is decided based on your working distance

If the working distance is less than 15 inches, 2.5x is a good choice.
If the working distance is about 16 inches to 18 inches, 3.0x or 3.5x may be selected.
If the working distance is larger than 19, 3.5x should be selected.

Many dental schools and hygiene schools recommend 2.5x loupes to their students as the 1st pair of loupes because the typical working distance of dental professionals in the past was about 14 inches, but 2.5x loupes are not adequate for students with long working distances.

Field of View and Selection of Loupe Optics

There are two kinds of loupe optics: Galilean loupes and Keplerian (often called Prism) loupes. If you want a larger field of view with high power loupes, you should choose prism loupes which offer a larger field of view than Galilean loupes.

Declination Angle for Safe Neck Posture

Small declination angles will force clinicians to excessively tilt their heads and can create chronic neck pain as a result of using non-ergonomic loupes with small declination angle for an extended period. A safe head tilt angle is one that is less than 20 degrees¹

Warning: Non-ergonomic loupes support human natural behaviors, such as tilting the head to look down rather than rotating eyes downward. Young clinicians may feel comfortable with non-ergonomic loupes because they will not feel the neck pain right away. If you want to protect your neck, you should evaluate your loupes within 45 days.

Maintenance Cost

The maintenance cost of loupes includes updating prescription (Rx), repairing unbonded loupes, and restoring working distance which can eventually increase due to aging eyes. The accommodation power of eyes decreases, requiring elderly individuals to use reading glasses to read books. Once we start to use reading glasses, the reading Rx changes frequently over time and we have to update Rx.

The maintenance cost of FLM loupes is the lowest because any optical shop can update eyeglass RX. The maintenance cost of TTL loupes is very expensive because updating the Rx of carrier lenses is the same as assembling TTL loupes. Even if you are only updating the Rx inside your loupe optics, this Rx update may take several weeks because your loupes will need to be returned to the manufacturers for the update to be performed. This requires a backup pair of loupes because it is not possible to practice without loupes.

Faucette says pediatric patients appreciate the variety of flavors as well. “Children tend to like non-mint flavors, so they usually like the Bubblegum, Walterberry, and Orange Cream. They tend to gravitate toward those because they don’t have the spiciness of mint,” she says. “With Enamelast [varnish’s] non-mint flavors, I haven’t noticed kids saying it’s spicy at all. Even if it does end up on their tongue, they seem OK. We were using a different brand before, and if it got on their tongue, they were like, blahhh.”

“I would for sure use Enamelast [varnish] on me, because I know that it works, it’ll be palatable, and give me all the benefits I need in a varnish—plus, it’s sweetened with xylitol. I really like that,” Faucette adds.

Jeanie Gates, RDH, says flavor is a primary factor when choosing a fluoride varnish and she makes sure to try them herself before she uses them on anyone else. “Taste is a big part. If it’s awful, then no one is going to do it. So it has to taste good. But it also has to stay on [the teeth]. There are some other products that taste good too, but they’re off teeth in like half an hour.”

Enamelast varnish features a patented adhesion-promoting formula that enhances retention, providing superior fluoride release and uptake. “It’s absolutely the fluoride varnish I want used on me,” Gates says.

“Some of the other products I’ve used are very thick and yellow, they’re hard to spread and you can’t get it in the interproximal areas,” says Sheri Barton, RDH. “I like Enamelast [varnish] because you can get it into the interproximal areas near the gum line and it’s thin, so it spreads nicely.” She says it’s convenient for people who aren’t just heading home after their office visit. “It’s very thin and gives a nice clean coating, and it’s clear so patients can go back to work and not really notice it.”

Echoing Faucette and Gates, Barton gives a resounding,

A New, Fresh Design in Soft Tissue Retraction

While we all do our best to maintain good dental hygiene, a long dental procedure is nearly inevitable. The cheek retractors needed for lengthy chair time—and the resulting anxiety, muscle fatigue, and pinching—are just not up to the task. There are a lot of patients who can finally opt for a better experience.

“Some retractors ask too much from the muscles, the ligaments, and at the end of the procedure you get this fatigue,” says Dr. Rafael Beolchi, DDS, MSc, of São Paulo, Brazil. “There is no muscle fatigue with Umbrella.”

Dr. Beolchi has encountered countless cheek retractors during his more than 20 years of experience in esthetic and cosmetic dentistry. “When we’re relaxed our teeth are not in contact. Our relaxed position in the mouth is when the mandible is open maybe 2–3 mm. Periodontal ligaments are not being stimulated which is good, and all the muscles are relaxed.” The innovative design of Umbrella immediately won him over because it mimics that relaxed posture for the patient.

“There is no muscle fatigue with Umbrella.”

“I usually take baby steps when I test a new product. But I had a big case (etching, bonding, composite, etc.) including Umbrella and this case was huge—six upper anteriors, six lower anteriors—full veneers in one single day. Developing Ultradent’s VALO Curing Light

Neil Jessop, Ultradent’s Vice President of Research and Development, recently spoke about the evolution of the VALO curing light. “There’s a sense of urgency that I constantly feel to keep an eye on what is right and on staying advanced. It’s not a free ride.” Ever innovating, Ultradent most recently released the VALO® Grand curing light, which offers a 50% larger lens that measures a whopping 12 mm. This larger lens allows clinicians to cover and cure an entire restoration with the light’s highly efficient 395 nm–480 nm LEDs. The story behind the VALO light’s development, however, started over two decades ago, in 1997.

Neil Jessop recalls, “My connection to the VALO light started a long, long time ago, way before Ultradent even offered a curing light. In the late nineties we were conducting a lot of bonding studies, and the only kind of curing light offered on the market at the time was a quartz-halogen curing light. They were big and bulky, which made it difficult to access restorations in the interior of the mouth, and they were also incredibly fragile and not completely dependable. We, as a company, set out to create our own curing light just when blue LED light chips began to appear on keychains and small gadgets. We found them incredibly intriguing because blue LEDs, unlike red LEDs, which have been around since the 1960s, don’t generate heat. Using blue LEDs would eliminate the requirement for cooling fans, which only add to the bulk of a curing light.”

Jessop continues, “We put a good three strong years into research, and after creating numerous prototypes and going through an intense process of trial and error, we released our VALO curing light in 2009, which we were really proud of.”

Because Ultradent is a vertically minded and vertically integrated company (we do our own research and development, manufacturing, marketing, selling, etc.), the company took on the challenge of manufacturing the VALO curing light in-house. As part of its unique design, the company decided that the light would be manufactured and milled out of a single bar of aircraft-grade aluminum, which not only gave the VALO curing light its modern look, but also made it incredibly durable and difficult to break, thanks to its unibody construction. This solved another age-old problem with the other curing lights on the market.

The VALO curing light immediately set itself apart, including one very unexpected feature. “One of the things that made the VALO light different from any other curing light at the time was the fact that we kept it corded. We did it this way because cordless curing lights, until then, had been known for being very expensive and hard to find batteries for. We wanted dentists to be able to rely on their curing light and not encounter that problem and we hadn’t found a way to solve that issue yet, so we kept it corded,” he says. “Not having a battery in the unit also made the VALO light lighter, sleeker, and smaller.”

Jessop recounts, “After we released the VALO Corded light, we began the process of developing the VALO cordless light, with the mission of making the batteries easily accessible [not exclusive to the VALO curing light] and low-cost. We discovered that we could use the same type of battery that is found in many digital cameras and other small devices.”

Thus, Ultradent introduced their VALO Cordless curing light. “I think the cordless light really spoke to our customers and showed them that we were in tune with them, we were catering to their needs, and that we were really listening.” Jessop says. “It really carved out a space for us in the curing light industry that had not been served and I’m so proud of our entire team for their hard work and innovation and the great lengths they went to make sure that quality went into every single painstaking detail. Our VALO curing light is amazing on every front, and that speaks volumes to everyone who went the distance to get it right.”

The VALO Corded and VALO Cordless lights went on to win numerous industry-wide awards year after year, including several Townie Choice and Pride Institute “Best of Class” and Cellerant technology awards. Ultradent later came out with the VALO Colors curing light line—a fun new take on the cordless light.

And so we come full circle to the release of the VALO Grand curing light, which Jessop says “solved clinical needs in a big, big way. The larger lens, which is 12 mm and is unsurpassed when it comes to other curing light offerings in the industry, allows the curing light to cover an entire restoration without leaving weak spots that often happens with smaller lenses. These weak spots often lead to restoration failure. The next largest lens on a curing light is only 9 mm and doesn’t offer the low profile head that our lights feature, which enables dentists to reach and cure restorations in the very back of the mouth.”

However, even though they’ve come far, Jessop says that the company and his research and development team aren’t resting on their laurels. “We didn’t get here for free. A lot of people worked so hard to get to this point, and I always feel a sense of urgency to stay ahead of the pack. There are a lot of smart people in this industry, so we try to maintain a certain level of vigilance in always looking for ways to improve and offer the best technologies out there,” Jessop remarks.

He also credits the company’s hallmark innovations, including the development of the VALO curing light family, to Dr. Dan Fischer, Ultradent’s founder and CEO. “Dr. Fischer has always been able to see the opportunities out there in the dental industry because he’s a clinician himself.
